Domestic Flight Unaccompanied Minor help?

So my 17 yr old is flying alone on United Airlines and I was wondering if I still have to fill out an 'Unacommpanied Minor Request Carriage' form for my 17yr old.

Unaccompanied minor service is not available for children over age 15. Children ages 16 and 17 may travel alone on any United- or United Express-operated flights.
